Health and Fitness Talk ? Hashimoto's disease

Hashimoto?s Disease also known as chronic lymphocytic thyroiditis is a?autoimmune disease?effecting the thyroid gland.? The thyroid gland sits in the front of the throat just below the ?adams apple.? ?It is responsible for the production of the hormones that regulate the metabolic processes in our bodies including temperature, heart rate and metabolism.

Hashimoto?s disease is the most common cause of hypothyroidism.? When hypothyroidism occurs the thyroid gland becomes enlarged, known as a goiter, and there is a gradual loss of function.

Hashimoto?s disease tends to run in families and is most commonly diagnosed in middle-aged women.? However, it can affect younger ages as well ? as I learned first hand. When I was 8 years old, my teacher, who also happened to be the principle of the little school I attended, came walking up my driveway one Sunday after church.? I was sitting outside under a tree resting when I saw her and I distinctly remember the sick feeling I had when she asked if my mother was home.? Wanting to lie, but knowing better I told her where my mother was.? Approximately 10 to 15 minutes later she left smiling and saying good-bye.? Within minutes my mother called me into the house. Wondering what I had done I headed for the door.? When I got inside, shaking on the inside, my mother started looking at my neck and feeling the area around my adams apple.? Then she looked at me and said she?d call the doctor in the morning.? I still had no idea what was going on, but Monday afternoon I was at the pediatricians office and from there was admitted to the hospital.? At that time no one had ever heard of an 8 year old having Hashimoto?s disease.

The symptoms of Hashimoto?s disease vary from patient to patient and can go unnoticed especially if the disease progresses slowly.? Some of the more noticeable symptoms, like the ones my teacher noticed, are the appearance of an enlarged thyroid gland, known as a goiter.? It can be enlarged to the point where you can?t see the adams apple and the neck can be red in appearance.? Chronic fatigue and decreased cognitive function, feeling cold even when it?s warm and facial swelling are also common.

Today, Hashimoto?s disease can be treated with thyroid replacements such as levothyroxine (synthroid).? In the mid 1960?s treatment was not as easy.? For one thing, the medications available today were not available then and those that were available had not been used on an 8 year old, according to my pediatrician.? At that time, the focus of treatment was on iodine and iodine replacement so my pediatrician told my parents they needed to get me to eat as much iodized salt as they could.? To this day I can?t eat meals without salt.

Left untreated Hashimoto?s disease can lead to other complications including muscle weakness and failure including heart failure.? It can also disrupt growth in children and adolescents, especially girls,require close monitoring as the thyroid gland controls the hormones that regulate the reproductive system causing the menstrual cycle to be irregular.? I had been told by my Drs that it was very unlikely that I would be able to have children.? Fortunately with the improved medications available i was able to have children.? With early detection and treatment you can lead a normal life, though you will always have to have your thyroid function monitored for any needed adjustments in medication.

Enroll in a four-year college degree of any major. You can either earn a Bachelor?s Degree in paralegal studies or with a legal assistant certificate or an associate degree in legal assistance from a community college. Other forms of relevant educational attainment will be an advantage.

Get certified. Although many firms do not require it, having a certificate in paralegal studies can place you at an advantage over rival applicants. Obtaining a paralegal certification will require passing a written exam to establish adequacy in legal knowledge and skill as well as paying a fee. Those who pass the exam can use the credential of Certified Legal Assistant or Certified Paralegal.

Apply for paralegal jobs at several law-oriented firms. Private firms, community legal services, district attorney?s offices and government organizations are all places that hire paralegals. Search for these offices using your school?s career-placement program or on the Web. Some firms have training programs for resident paralegals and may hire applicants with no legal experience, although those specifically looking for paralegals will favor applicants with experience in this area of the law.

Stay updated on new paralegal techniques and regulations. Renewing your certificate after a certain number of years will require continuing education credits, either from classroom lessons or online schooling. There is also an Advanced Paralegal Certification for those who wish to specialize in criminal law or other sub-fields.

Advance in your career. Paralegals can be promoted to a supervisory or managerial level where they oversee other paralegals and organize their department. These advancements will come with a salary and benefits increase.

As a criminal law paralegal, you cannot legally dispense legal advice or represent a client in court, no matter your level of knowledge with regards to the law. Make sure the clients you encounter understand this distinction in paralegal work and refuse any invitation to perform legal representation.

Iron Man Recast? Robert Downey, Jr 'Would Really Hate' That

The actor tells MTV News he expects to play the superhero 'for a long while.'

With the Avengers finally assembling next week, Marvel Studios and the stars behind the heroes have already begun to look ahead to the next films set in the comic-book universe. At the closing night celebration of the Tribeca Film Festival on Saturday night (April 28), Robert Downey, Jr. spoke with MTV News about the future of his character, Iron Man, and addressed recent talk about other actors playing the role in the future.

At CinemaCon last week, Marvel Studios president Kevin Feige mentioned to Badass Digest that when it came time for Downey to leave the role of Tony Stark behind, a new actor could pick up right where he left off, like in the James Bond films.

But Downey told MTV News on the red carpet that he wasn't so thrilled about the idea of someone else playing Tony Stark. "I would really hate for someone else to think they could step into my shoes," he said. An infamous jokester, he couldn't help but ask, "I wonder who's playing Tony Stark next. Is it you? Is that what you're here to tell me?"

But Downey can't be Iron Man forever, and while the 42-year-old actor couldn't say for sure how many movies he has left in him, he made clear he'll be interested as long as they remain good films.

"I know things are looking like that they could go on for a long while," Downey said. "As usual in my book, it's all about quality control and delivering a product that you can have this kind of reaction to. If they keep doing that, maybe I'll keep showing up."

In Downey's immediate Marvel future, there's next year's "Iron Man 3." The past few weeks have seen actors like Sir Ben Kingsley, Guy Pearce and Jessica Chastain potentially joining the cast, and Downey said that he could not wait to work with "any or all of them."

"It's funny. Sometimes you have to call for a good old-fashioned British invasion," Downey said. "'Iron Man 3' is really shaping up to be something special. We start in a couple weeks. I'm sure I'll be leaking secrets to you as we move ahead."

What secrets, you ask? Well, it was recently announced that "Iron Man 3" would be co-produced in China. As for whether any filming would take place in the East, Downey was less forthcoming. "I'd like to tell you all my secrets," he said," but I forget them."

March is colorectal cancer awareness month. Colorectal cancer is the fourth most common cancer among men behind skin, prostate and lung cancer, and the fourth most common among women after skin, breast and lung cancer. The risk of colorectal cancer increases with age with more than 90 percent of cases occurring in people who are 50 years old or older (CDC).?

According to the NCI, risk factors for colorectal cancer are:

  • being 50 years old or older, ?
  • ?having a family history of colon cancer or polyps,
  • colorectal polyps,
  • a personal history of cancer, especially ovarian, endometrial or breast cancer,
  • ulcerative colitis or Crohn?s disease,
  • hereditary conditions like familial adenomatous polyposis (FAP) and hereditary nonpolyposis colon cancer (HNPCC),

It is important to get tested for colorectal cancer if you are 50 years old or older because it can save your life. Many cancers in the early, treatable stages do not cause symptoms so it is important to get tested regularly. Symptoms usually begin to emerge when the cancer has progressed. Some of the symptoms of colorectal cancer include:
  • Constipation or Diarrhea
  • Blood in or on the stool
  • Weight loss with no known reason
  • Fatigue
  • Vomiting or nausea
  • The sensation that the bowel does not empty completely
  • Narrow stools (which indicate an obstruction in the colon)
  • Chronic abdominal discomfort, like gas pains or cramps or feeling bloated or full

West Antarctic ice shelves tearing apart at the seams

ScienceDaily (Mar. 27, 2012) ? A new study examining nearly 40 years of satellite imagery has revealed that the floating ice shelves of a critical portion of West Antarctica are steadily losing their grip on adjacent bay walls, potentially amplifying an already accelerating loss of ice to the sea.

The most extensive record yet of the evolution of the floating ice shelves in the eastern Amundsen Sea Embayment in West Antarctica shows that their margins, where they grip onto rocky bay walls or slower ice masses, are fracturing and retreating inland. As that grip continues to loosen, these already-thinning ice shelves will be even less able to hold back grounded ice upstream, according to glaciologists at The University of Texas at Austin's Institute for Geophysics (UTIG).

Reporting in the Journal of Glaciology, the UTIG team found that the extent of ice shelves in the Amundsen Sea Embayment changed substantially between the beginning of the Landsat satellite record in 1972 and late 2011. These changes were especially rapid during the past decade. The affected ice shelves include the floating extensions of the rapidly thinning Thwaites and Pine Island Glaciers.

"Typically, the leading edge of an ice shelf moves forward steadily over time, retreating episodically when an iceberg calves off, but that is not what happened along the shear margins," says Joseph MacGregor, research scientist associate and lead author of the study. An iceberg is said to calve when it breaks off and floats out to sea.

"Anyone can examine this region in Google Earth and see a snapshot of the same satellite data we used, but only through examination of the whole satellite record is it possible to distinguish long-term change from cyclical calving," says MacGregor.

The shear margins that bound these ice shelves laterally are now heavily rifted, resembling a cracked mirror in satellite imagery until the detached icebergs finally drift out to the open sea. The calving front then retreats along these disintegrating margins. The pattern of marginal rifting and retreat is hypothesized to be a symptom, rather than a trigger, of the recent glacier acceleration in this region, but this pattern could generate additional acceleration.

"As a glacier goes afloat, becoming an ice shelf, its flow is resisted partly by the margins, which are the bay walls or the seams where two glaciers merge," explains Ginny Catania, assistant professor at UTIG and co-author of the study. "An accelerating glacier can tear away from its margins, creating rifts that negate the margins' resistance to ice flow and causing additional acceleration."

The UTIG team found that the largest relative glacier accelerations occurred within and upstream of the increasingly rifted margins.

The observed style of slow-but-steady disintegration along ice-shelf margins has been neglected in most computer models of this critical region of West Antarctica, partly because it involves fracture, but also because no comprehensive record of this pattern existed. The authors conclude that several rifts present in the ice shelves suggest that they are poised to shrink further.

The NFL may not be interested in exploring the depths of the intentional-injury rabbit hole, but the disclosure of a bounty system in New Orleans and multiple reports of similar programs elsewhere has created a feeding frenzy when it comes to accounts of teams encouraging players to attempt to hurt opponents.

The latest allegation comes from former Vikings defensive lineman Brad Culpepper. who claims that, in the 1990s, the defensive linemen were coached on the finer points of knocking the quarterback out of the game.

?We practiced techniques to injure players,? Culpepper said on Mike Bianchi?s Orlando-based radio show, via Jeremy Fowler of the St. Paul Pioneer Press.? ?We would dive on a mat and aim for the knee portion of a pad like it was the quarterback, and we would practice going at that and trying to bust the quarterback?s knee. You knock the quarterback out, you win the game. . . .? If you pull on their arm and land on their shoulder, you can dislocate [the shoulder] and then they?re out.? We used to practice that, too.?

Former Vikings defensive end Chris Doleman disagrees.? ?That wasn?t a part of it,? Doleman told Fowler.? ?I would know.? I was a starter.? It was never about hurting a quarterback.? Intimidating a quarterback, knocking him around ? absolutely.? One hundred percent.? But not injuring. . . .

?Maybe why [Culpepper's] going in that direction is this, for example ? when you try to get a quarterback down, the way to strip a ball is to get his arms straight,? Doleman said. ?If you look at film, when I?m going after a quarterback, we put our arms down on his arms, like we?re putting him in a bag, so he has no control over the ball.? Can?t throw it.? But that?s proper technique, nothing else.? Our thing was, cause a disruption in the backfield ? but to get the ball back, not to hurt.?

Culpepper contends that no money was offered for inflicting injury on quarterbacks.? ?I would have done anything to gain favor with the coaches or my teammates,? Culpepper said.

Complicating matters is that, from 1992 through 1995, the Vikings? defensive coordinator was Tony Dungy, who enjoys a stellar reputation throughout league and media circles.? But the defensive line coach from 1992 through 1994 was John Teerlinck, who found himself summoned to the league office in 1996 for a meeting with Commissioner Paul Tagliabue regarding Teerlinck?s reputation for instructing defenders to go low on quarterbacks.

Thus, while it?s possible that Teerlinck was teaching defenders to aim low and/or to drive shoulders into the ground, it undoubtedly would have been something that happened without Dungy?s knowledge or involvement.

Indeed, Culpepper spent only two seasons with the Vikings (1992 and 1993) before joining the Buccaneers, where Dungy later became head coach.? Given that Culpepper worked with Dungy for four years in Tampa and only two in Minnesota and given that Teerlinck never coached for the Bucs and given that Culpepper isn?t claiming that the same practices were used in Tampa, it?s safe to assume that, if anything dirty was going on in Minnesota, Teerlinck was teaching it.

UPDATE 2:53 p.m. ET:? In the full radio interview, Culpepper specifically mentions Teerlinck.? Culpepper never mentions Dungy.? Culpepper also says that ?none of this stuff went on in Tampa.?

(AP) ? Charges filed Friday against Army Staff Sgt. Robert Bales reflect the horror of the crime: 17 counts of premeditated murder, more than half of them children, during a shooting rampage in southern Afghanistan. But while Afghans are calling for swift and severe punishment, it will likely be months, even years, before the public ever sees Bales in a courtroom.

One only has to look at two recent and similarly high-profile cases to see that the wheels of military justice turn slowly.

It's been nearly 29 months since an Army psychiatrist, Maj. Nidal Hasan, allegedly killed 13 and injured two dozen more at Fort Hood, Texas. His trial is scheduled to begin in June. And it's been 21 months since the military charged intelligence analyst Bradley Manning with leaking hundreds of thousands of pages of classified information. It took nine months before he was deemed competent to stand trial.

The Bales case is likely to be equally complex, involving questions of his mental state and the role that the stresses of war and possible previous head injuries may have played in his alleged actions. Most of the eyewitnesses are the Afghan villagers and survivors who may be brought in for the trial.

The military on Friday charged Bales with 17 counts of murder, six counts of attempted murder and six counts of assault in the March 11 pre-dawn massacre in two southern Afghanistan villages near his base. The father of two from Lake Tapps, Wash., was officially informed of the 29 charges just before noon at the U.S. military prison at Fort Leavenworth, Kan., where he is confined.

The maximum punishment for a premeditated murder conviction is death, dishonorable discharge from the armed forces, reduction to the lowest enlisted grade and total forfeiture of pay and allowances, according to Col. Gary Kolb, a spokesman for U.S. forces in Afghanistan. The mandatory minimum sentence is life imprisonment with the chance of parole.

The charges offered few details of what happened that night. But the 38-year-old soldier is accused of walking off his base with his 9mm pistol and M-4 rifle, which was outfitted with a grenade launcher, killing four men, four women, two boys and seven girls and burning some of the bodies. The ages of the children were not disclosed.

In the most detailed descriptions of the shootings to date, the charges say Bales shot a young girl in the head, a young boy in the thigh, a man in the neck and a woman in the chest and groin. The documents also say that he "shot at" another girl and boy, but apparently did not hit them.

The attack occurred in the Panjwai district of Kandahar Province, the spiritual birthplace of the Taliban. The dead bodies were found in Balandi and Alkozai villages ? one north and one south of the base.

Members of the Afghan delegation investigating the killings said one Afghan guard working from midnight to 2 a.m. saw a U.S. soldier return to the base around 1:30 a.m. Another Afghan soldier who replaced the first and worked until 4 a.m. said he saw a U.S. soldier leaving the base at 2:30 a.m. It's unknown whether the Afghan guards saw the same U.S. soldier. If the gunman acted alone, information from the Afghan guards would suggest that he returned to base in between the shooting rampages.

It also is not known whether the suspect used grenades, Kolb said. The grenade launcher attachment is added to the standard issue M-4 rifle for some soldiers but not all, he said. Bales was assigned to provide force protection at the base.

The case against him is the worst allegation of killing of civilians by an American in Afghanistan and has severely strained U.S.-Afghan ties at a critical time in the decade-old war.

The Taliban said they had no faith that the U.S. military court system would bring the shooter to justice. Taliban spokesman Zabiullah Mujahid said in an email to The Associated Press that the insurgents themselves would avenge the killings by killing U.S. forces one-by-one.

Not addressed in the charges are suggestions that Bales may have been drinking. On Friday, a senior U.S. defense official said Bales was drinking in the hours before the attack on Afghan villagers, violating a U.S. military order banning alcohol in war zones. The official spoke on condition of anonymity to discuss the charges before they were filed.

U.S. officials, however, have said that additional charges could be filed as time goes on. Bales' civilian attorney, John Henry Browne, said Friday the government will have a hard time proving its case and that his client's mental state will be an important issue. Bales was on his fourth tour of duty, having served three tours in Iraq, where he suffered head and foot injuries.

Meanwhile, an attorney for Bales' wife said the couple have spoken by telephone twice since he was detained. Attorney Lance Rosen said Bales called his wife, Karilyn, first from overseas shortly after the March 11 pre-dawn massacre, then from Fort Leavenworth on Wednesday.

Rosen said Bales and his wife were warned the call Wednesday would be monitored and told they would only have 10 minutes to talk. They talked about family matters and "reaffirmed their love for each other," Rosen said. He also said the family has set up a defense fund to help pay for Bales' legal fees.

The charges set in motion what will likely be a lengthy military justice process.

"Usually in military and civilian criminal cases, delay accrues to the benefit of the accused. So there will be some maneuvering to put it off a while," said Gary Solis, retired Marine prosecutor and adjunct law professor at Georgetown University. "I think it's many months before we're going to see this thing go to trial, if it ever goes to trial."

Bales may face what the military calls a "sanity board," to determine his mental state at the time, and whether he is competent to stand trial.

Since Bales was assigned to a unit based at Joint Base Lewis-McChord in Washington state? the 2nd Battalion, 3rd Infantry Regiment of the 3rd Stryker Brigade Combat Team of the 2nd Infantry Division ? the charges were sent Friday to a special court-martial convening authority, the 17th Fires Brigade, an artillery unit at the post. Lewis-McChord spokesman Lt. Col. Gary Dangerfield said officials at the post will have the legal responsibility of trying and managing the case against Bales, but it was not clear where the proceedings would actually take place.

Maj. Christopher Ophardt at Lewis-McChord said the preliminary hearing may not take place for a few months and it would likely be several more months after that before a trial would begin ? perhaps two years from now depending on motions and other pretrial actions.

Jeffrey Addicott, who previously served as the senior legal adviser to the U.S. Army's Special Forces, said the military justice system typically moves faster than civilian courts. But he said the international political implications may cause this case to drag on for years.

"When we have these high-publicity cases, the military becomes like a deer in the headlights. They have just one speed: extremely slow," Addicott said, adding that he expects the defense team to use all tactics to slow the process.

According to military lawyers and experts, now that the charges have been filed, the next step is for the military is to decide whether there is enough evidence to refer the charges to a preliminary hearing. That hearing, what the military calls an Article 32, would determine whether there's probable cause to believe a crime was committed and that the person charged did it.

Once the Article 32 is over, the hearing officer recommends to a general officer whether or not the case should go to trial. That general officer then decides whether the case should go to a court-martial in front of a military judge.

Throughout the proceedings, the lawyers will be able to file a wide range of motions, including requests for more time to review evidence.

Browne said he thinks the U.S. government will have difficulty proving its case against Bales because "there is no crime scene" and a lack of important physical evidence like fingerprints. And he has said he wants to visit Afghanistan.

Prosecutors, meanwhile, will rely on evidence collected from the villages, including any statements by witnesses and Afghan civilians who were injured that night, as well as any observations by other soldiers about Bales' conduct and his movement on or off the base.

Two military defense attorneys also have been assigned to his case.

Ophardt said Bales' family is still living on base but has the option of moving off at any time.
